The shirt shows Michelangelo’s Creation of Adam repainted — the reclining figure reaching up, and reaching back from the red cloak, a figure with a pig’s head.
It is painted in imitation of the fresco, right down to the colouring and the pose, and there is no text anywhere.
The joke belongs to a corner of the internet where a specific pig-headed character has acquired a mythology. Applying that to the ceiling of the Sistine Chapel is the escalation.
